Licensed Practical Nurse (LPN) Home Health Full-time Job

Job description

• Monitoring basic patient health such as vital signs and overall condition
• Changing dressings or inserting catheters
• Taking patient histories and maintaining documentation
• Assisting with tests or procedures
• Providing personal care, such as helping with bathing and toileting
• Consulting with RNs on care plans
• Informs Registered Nurse (RN) designee of changes in patients’ condition.
• Taking medical histories and entering information into computer systems
• Monitor patient’s fluid and food intake/output while also managing their IVs
• Giving medications as prescribed by a physician, including injections and immunizations
• Proven experience as LPN or similar role
• Ability to undertake a variety of diverse care tasks and work in a multidisciplinary team
• Familiarity with home health, safety and sanitation standards and procedures
• Understanding of confidentiality obligations and nursing best practices
• Medicaid Documentation

Requirements

*AHCA level 2 fingerprinting

*Physical within the last 6 months

*Current TB test or Current chest x-ray

*Valid Driver’s license

*Social Security Card

*Current CPR Card

Minimum 1 Year Medicaid/Oasis Documentation Experience Preferred
