Job Summary

Ennis Healthcare Solutions, a Home Healthcare Agency, is currently seeking a Physical Therapist to evaluate patients requiring Physical Therapy as treatment in the Miami Dade County area.

Responsibilities and Duties

Services provided by the Physical Therapist shall be performed within the scope of practice authorized by the license issued by the State for the practice of Physical Therapist.

  • Candidate shall provide Physical Therapist services as prescribed by a Physician, Physician Assistant, or advanced registered Nurse Practitioner, acting within their scope of practice, which can be safely provided in the home.
  • Conducts regular or OASIS assessments accurately, according to instructions in the OASIS Implementation Manual, and corresponding to documentation contained elsewhere in the assessment role.
  • Participate in in-service program, and discharge planning.
  • Candidate shall provide Physical Therapy services, and evaluation of equipment needs as applicable.
  • Candidate shall instruct the patient's family on the patient's total physical therapy program.
  • Able to speak, read and write in English.
  • Able to read assignments, follow directions.
  • Able to communicate and respond clearly via telephone and respond toe patient's spoken needs.
  • Ability to physically transfer, lift or assist patients whose average weight is 160 pounds with or without the aid of mechanical devices.
  • Able to spend 80% of the work standing and/or moving about.
  • Able to walk, climb stairs, stoop, twist, bend and squat to perform essential job functions.
  • Able to concentrate on detail with frequent interruptions.
  • Able to follow, complete and remember daily routines and requirements.
  • Able to comprehend and utilize professional education materials.
  • Able to cope with the mental and emotional stress of the position.
  • Ensure HIPAA guidelines and procedures and maintained.
  • Perform evaluation and assessment.

Qualifications and Skills

  • Candidate must be a graduate of an approved school for Physical Therapist.
  • Must be currently licensed in the state of Florida.
  • One year of experience preferred.
  • Valid CPR card
  • Level 2 Background
  • 2 Hour Alzheimer training
  • Statement of Health
  • Liability Insurance
  • Must have all documentation up to date: professional license, driver's license, CPR, CEU's, Physical, and background check (ACHA).

Job Type: Contract

Pay: $60.00 - $70.00 per hour
