LifeCare Solutions of Palm Beach, Inc. is a Medicare Home Health Agency servicing the Palm Beach County areas for over 14 years!

We are now hiring Per Diem Registered Nurses for Medicare Home Health visits and OASIS assessments in PALM BEACH COUNTY.

Medicare OASIS experience and one year RN home health experience mandatory!

Registered Nurse (RN) Requirements;

· Current state license as a Registered Nurse

· Minimum 2 years nursing experience

· Kinnser EMR system experience preferred

· Current PPD or Chest Xray

Registered Nurse (RN) Responsibilities:

· Complete initial assessment (OASIS)

· Initiate and develop plan of care

· Provide health care instructions to the patient

· Identify discharge planning needs as part of the care plan development and implements prior to discharge of the patient.

· Update & communicate with the PCP / CM / ALF when necessary or condition changes.

North Palm Beach County: Palm Beach Gardens, Lake Park, North Palm Beach, Jupiter, Juno Beach, and surrounding communities

Central Palm Beach County: West Palm Beach, Lake Worth, Riviera Beach, and surrounding communities

South Palm Beach County: Boca Raton, Delray Beach, Boynton Beach, Lantana, Lake worth, and surrounding communities

Western Palm Beach County: Loxahatchee, Royal Palm Beach, Wellington, and surrounding communities

Pay: $45.00 - $80.00 per hour
