Position Summary:
- Provides oversight of all patient/client care services and personnel.
- Ensures quality of patient/client care in the home.
- Coordinates care with the interdisciplinary team, patient/client, family and referring agency.
Detailed Functions:
- Makes patient/client and personnel assignments. Supervises or manages, directly or through qualified subordinates, all personnel who provide direct patient/client care.
- Coordinates patient/client care. Ensures communication with all physicians involved in a patient’s/client’s plan of care and other health care practitioners (as appropriate). Integrates orders from all relevant physicians involved into the plan of care and ensures the orders are approved by the responsible physician. Coordinates and integrates care provided by Agency staff, including contractual providers.
- Assures that patient/client needs are continually assessed. Assures reassessment of the patient’s/client’s needs is performed by the appropriate healthcare professional when there is a significant health status change in the patient’s/client’s condition, at the physician’s request, and after hospital discharge.
- Assures the development, implementation, and updates of the individualized plan of care in partnership with the patient/client, representative (if any), and caregiver(s).
- Ensures provision of services as ordered by the physician as indicated in the plan of care, including patient/client and caregiver education and patient/client, caregiver, and family counseling. Prepares clinical notes as appropriate.
- Assumes responsibility for continuity, quality, and safety of services delivered in compliance with State and Federal regulations.
- Participates in activities relevant to professional services including development of qualifications and assignment of Agency personnel. Participates in Agency-sponsored in-service training. Recommends resources needed to provide care.
- Collaborates on development and implementation of policies and procedures which guide and support the provision of care. Collaborates with Agency leadership and appropriate staff to develop the organizational plan as it pertains to patient/client care.
- Monitors and participates in the QAPI Program and assures appropriate corrective measures are performed. With the Governing Body, establishes and directs an ongoing quality assurance program which assures case assignment and management is appropriate, adequate, and consistent with the Plan of Care, nursing and other services provided to the patient are coordinated, appropriate, adequate, and consistent with the Plan of Care, all services and outcomes are completely and legibly documented, dated, and signed in the clinical service record, confidentiality of patient data is maintained, and findings of the quality assurance program are used to improve services.
- Monitors systems and identifies problem areas to Administrator both verbally and through written reports.
- Meets mandatory continuing education requirements of the Agency and licensing board.
- Establishes policies and procedures that are consistent with recommended Centers for Disease Control (CDC) and Occupational Safety and Health Agency (OSHA) guidelines for safety, universal precautions, and infection control procedures. Promotes and educates regarding the concepts of infection control and standard precautions in coordinating or performing patient/client care activities to prevent contamination and transmission of disease.
- Ensures that the professional standards of community nursing practice are maintained by all nurses providing care.
- Maintains and adheres to Agency procedures and patient/client care policy manuals.
- Establishes with the Administrator or Alternate Administrator, policies and procedures on biomedical waste for the agency providing nursing and physical therapy services (The Department of Health website has information on biomedical waste handling and the requirements at www.doh.state.fl.us/Environment/Community/biomedical.)
- Employs and evaluates nursing personnel.
Requirements:
Active Florida Nursing License
Bachelors in Nursing Preferred
1year of supervisor experience
Full time or Part Time available
