Primary Care of Central Florida was founded in 2005 and serves the Central Florida geriatric population. We are a large practice with over 25 Practitioners that serve the Skilled Nursing Facilities, Assisted Living Facilities and our Homebound patients in the comfort of their home in 7 counties. We are committed to providing excellent care to our patients and buildings while reducing readmissions into the hospital.
Our medical practice is looking for a few more outstanding full time Certified Medical Assistants to join our team in providing high quality medical care to our patients. This position requires a certification / license in the medical field to qualify.
Home Medical Assistant Responsibilities:
- Work with assigned Team
- Ensure census is properly maintained
- Call assigned patients as needed for Wellness Checks
- Complete a monthly medication reconciliation for assigned patients
- Provide care coordination, ensuring all needs, not just medical, are being met
- Thorough, accurate documentation of all activities
- Ensure patients have a scheduled upcoming PCP appointment and that it is confirmed by patient/family
- Schedule follow up appointments as needed
- Verify that Annual Wellness Exams are completed, or scheduled
- Complete AWE (Annual Wellness Exam) & LEHS (Lower Extremity Health Survey) Questions prior to scheduling AWE appointment
- Confirm Advanced Care Planning appointments are scheduled.
- Contact third parties, such as specialists and home health care agencies, to obtain notes for our providers to review
- Use company disease state guides to ensure patients receive excellent condition management
- Ensure workflow process is being completed daily (form given)
- Review charts for completion, and obtain needed updates/documents
Facility Medical Assistant Responsibilities
- Ensure Facility Census is properly maintained
- Communicate with provider(s) that rounds at facility
- Communicate with nurses and staff at the facility
- Review charts for completion, and obtain needed updates/documents
- Check in with patients for Wellness Checks
- Call assigned patient’s family/POA monthly and as needed for updates
- Complete a monthly medication reconciliation for assigned patients
- Provide care coordination, ensuring all needs, not just medical, are being met
- Thorough, accurate documentation of all activities
- Ensure patients have been seen in an appropriate time fame
- Communicate with ALF/SNF Scheduler for appointments as needed
- Verify that Annual Wellness Exams are completed
- Confirm Advanced Care Planning appointments are scheduled
- Contact third parties, such as specialists, home health care agencies, and hospice to obtain notes for our providers to review
- Use company disease state guides to ensure patients receive excellent condition management

Requirements :
- A high school diploma or GED.
- Certification in the medical field
- Proven work experience in the medical field
- Knowledge of medical office management systems and procedures
- Knowledge of MACRA/MIPS/HEDIS preferred
- Excellent time management skills with ability to multi-task and prioritize work
- Excellent written and verbal communication skills
- Strong organizational and planning skills
- A positive and professional attitude.
- Maintain a clean and neat appearance at all times.
- Have the ability to work with precision under pressure.
- Proficiency use of Microsoft Office Suite, including word and excel.
- Ability to operate office equipment, printers, fax machines, and scanners.
- Bilingual preferred
- Reliable transportation
Job Types: Full-time, Contract
Pay: $15.00 - $21.00 per hour
