Case Manager Full-time Job

REQUIREMENTS:

• Bachelor’s Degree
• 2 years minimum of relevant experience
• Must be willing to travel within their region

PLUSSES:

• Long term care or managed care experience
• Bilingual in Spanish

DIFFERENCES:

• NO COVID vax requirement (still need vehicle, DL, and ability to pass BG & drug)
• NO tech assessment required
• Will spend 75-80% of time in field doing field visits. 20-25% remote admin/coordination tasks
• ONLY supporting elderly/long term care patients (includes some disability) but no homeless, special needs, substance abuse, etc.
• Will cover a larger territory so most of their members will be in their county but they MUST be willing to travel to any part of their region when needed
• Case loads will be MAX: 60 if individual home based members, up to 80 or 100 if assisted living facility or nursing home based (multiple members in the same facility)

Job Type: Full-time

Pay: $45,000.00 - $58,000.00 per year

Education:

• Bachelor's (Required)

Experience:

• Case management: 3 years (Preferred)
• Long Term Care: 1 year (Preferred)
