Position Title – Care Coach 1
Address - Indian River, Martin, Okeechobee, Palm Beach, and St. Lucie FL - FIELDWORK Required
Pay -- $25.65
Start date – 12/5/22
Hours – Mon-Fri; 8:30 AM – 5 PM EST
Vaccine Requirement – A vaccine is required

Required Skills & Education:

• Must Have Experience in Case Management, Coordinating Services, and Conducting Assessments to Determine Needs
• Care Coaches with the following qualifications shall also have a minimum of two (2) years of relevant experience:
• Bachelor’s degree in social work, sociology, psychology, gerontology, or a related social services field;
• Bachelor’s degree in a field other than social science; or Care Coaches with a master’s degree in social work, sociology, psychology, gerontology, or a related social services field may substitute experience obtained through a practicum, internship, or clinical rotation on an equivalent basis for up to one (1) year of the experience requirements
• Care Coaches with the following qualifications shall also have a minimum of four (4) years of relevant experience: Licensed Practical Nurse, licensed to practice in the State
• Care Coaches without the aforementioned qualifications may substitute professional human service experience on a year-for-year basis for the educational requirement
• Case Managers without a bachelor’s degree shall have a minimum of six (6) years of relevant experience
• 2+ years of experience with electronic case note documentation and experience with documenting in multiple computer applications/systems
• 2+ years of computer skills and experience with Microsoft Word, Excel and Outlook
• Bilingual (Spanish) Strongly Preferred
• Registered nurse is preferred but not required

Responsibilities :

• Visit Medicaid members in their homes, Assisted Living Facilities, and/or Long Term Care Facilities and other care settings – 75-90% local travel
• Assesses and evaluates member's needs and requirements in order to establish a member-specific care plan
• Ensures members are receiving services in the least restrictive setting in order to achieve and/or maintain optimal well-being
• Planning and implementing interventions to meet those needs
• Coordinating services, and monitoring and evaluating the case management plan against the member's personal goals
• Guides members/families toward resources appropriate for their care
• Services are driven by facilitating interactions with other payer sources, providers, interdisciplinary teams, and others involved in the member’s care as appropriate and required by our comprehensive contract
