In this role, you’ll be advocating for older adults and their loved ones to make decisions related to healthcare, aging, and long-term care planning to enhance their overall quality of life. You’ll have the opportunity to assess each client and family situation by evaluating the client’s level of functioning, health, emotional state, finances, and legal needs to advocate and provide the best possible recommendations to improve their quality of life.

Duties and Responsibilities:

Home Care Duties

  • Provide orientation and training to new and existing caregivers
  • Manage intakes of new clients and ensure successful starts of care.
  • Meets with new clients and completes on-site safety assessment and care plan.
  • Precept and qualify new aides in the field, recording and documenting their skill abilities and safety concerns addressed.
  • Provides safety training and coaching to new and current caregivers during in-home assessments
  • Provides coaching for caregivers with a new client to ensure a successful experience for both caregivers and clients
  • Completes on-site assessments to be used for caregiver’s Caregiver Coaching Opportunity meetings
  • Responsible for professional behavior on behalf of clients and in all community venues.

Care Management Duties

  • Act as an advocate for clients, recognizing the client's values rather than their own.
  • Conduct and develop care plans and assessments to identify problems, eligibility for assistance, and need for services.
  • Develop, monitor, and manage the care plan.
  • Onboard and sign up new clients educating them on how our services will help them age in place.
  • Screen, arrange, and monitor all services to optimize benefit to the client.
  • Review financial, legal, or medical issues and offer referrals to specialists to avoid future problems and conserve assets.
  • Identifying resources to address clients’ health, daily living, and safety needs.
  • Assist with determining the level of care needed and moving a client to an appropriate facility or arranging appropriate in-home care.
  • Accompanying clients to provider and/or physician appointments.
  • Monitoring clients’ health status and well-being.
  • Act as a liaison to responsible parties and related professionals, local or distant, to keep them informed of the client's situation.
  • Complete documentation requirements accurately and timely.
  • Participate in marketing efforts to educate the public, cultivate clients, and securing new clients.

Job Type: Full-time

Pay: $60,000.00 - $67,000.00 per year
