Physical Therapist / PT- Home Health - PRN - 1099 (Lymphedema a PLUS**) Full-time Job

This position is a PRN position 1099 position.

Lymphedema Certification is a plus and offers higher pay per visit.

As an independent contractor, you have the right to accept or decline patients according to your availability.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

• Ensure delivery of high-quality patient care services.
• Development of the Plan of Care, this may include performing Oasis Start of Cares, Evaluations , Reassessments and DC
• Occasionally you may treat patients if a PTA is not available to provide treatments
• Ensure proper documentation of patient care.
• Implement effective communication with own and other departments' staff, with medical staff and patients to ensure coordinated efforts and provision of high quality service and optimal patient care.
• Plan, design, implement and interpret specific physical therapy treatment programs for patients to facilitate the patient's recovery and attainment of maximum physical potential.
• Educate patients in the proper care and use of supports and performance of exercise programs.
• Perform or delegate patient treatment to include modalities, therapeutic exercises, gait training and hydrotherapy.
• Follow appropriate documentation and reporting procedures including case conferences
• Work closely with the physician and HHA to ensure comprehensive delivery of services and quality patient care.

EDUCATION: Degree from an APTA-approved school of physical therapy and approved by the Physical Therapy Examining Committee of the State of FL.

REQUIREMENTS:

• Minimum of six months home health experience
• Current CPR certification
• Current physical stating free of communicable diseases and able to perform your job as a PT.
• 2 Step PPD, chest X-ray or TB blood test
• Good organizational and communication skills
• This role requires full COVID-19 vaccination or notice of exemption
• Must be proficient in Wellsky, Homecare Homebase, Axxess and Kantime EMRs or must be willing to learn.
• Bilingual is a plus!

Job Types: Full-time, Part-time

Pay: $80.00 - $95.00 per hour
