I. Summary

The Nurse Coordinator is responsible for the direct and indirect personal care services given to residents.

II. Qualifications

· Must be a graduate of an accredited school of nursing and must have and maintain a valid, unrestricted nursing license in the state.

· Experience with aging adults and knowledge of regulations pertaining to Assisted Living Facilities preferred.

· Individual must also be able to read, write, and speak the English language and be able to communicate effectively with residents, family, physicians and staff.

· Alertness and skill in recognizing and identifying symptoms.

· Ability to adhere to acceptable time frames for routine matters and assignments while having constant interruptions.

III. Physical Demands

Requires moderately heavy physical exertion on a regular and recurring basis. Frequently required to stand, walk, reach with hands and arms, sit, stoop, kneel, crouch, crawl, talk, and hear. Occasionally required to lift and/or move up to fifty pounds. Specific vision abilities include close, peripheral, depth perception, and ability to focus.

IV. Working Conditions

Assisted living community and general office environment. Potential and/or direct exposure to blood, body fluids, infectious diseases, air contaminants, and hazardous chemicals.

Working beyond normal working hours, on weekends and holidays, in other positions temporarily, if necessary. Scheduled hours may change to depending on the needs of the facility.

V. Essential Functions

1. Provide direct resident services and medication management when needed.

2. Ensures quality and safe delivery of services by participating in development and implementation of resident service plan to implement goals designed to promote the maximum level of functioning for each individual in the assisted living setting.

3. Oversee the provision of assistance with ADL’s to residents whose service plan indicates the need

4. Documents in the resident records, in an accurate and timely manner, to reflect the delivery of quality and safe services designed to meet the needs of the resident.

5. Assisting care providers and ensuring continuing promotion of the resident's physical and emotional health by assisting and teaching the resident regarding his/her medical care, (documents if resident refuses to follow plan of service in resident's chart).

6. Schedule tests ordered by the physician, assist residents in scheduling medical appointments and transportation.

7. Visit/assess residents who are ill and provide/arrange necessary follow-up.

8. May assist with orientation and education of resident care staff.

9. Make daily rounds to monitor care provided by resident care staff.

10. May assist with scheduling and supervision of resident care staff.

11. Keep MAR’s complete and accurate.

12. Oversee medication management for residents whose service plan demonstrates the need.

13. Demonstrate competency in all areas of medication administration.

14. Comply with and maintain current knowledge of community, state and licensure guidelines/standards of practice.

15. Promptly notifies the resident’s health provider of any medication errors, omissions, side effects and/or food-drug interactions.

16. Maintain medication storage, destruction and narcotic management within the community and regulatory guidelines.

17. Participates in resident service plan conference as appropriate.

18. Observes that resident conditions are accurately reported to the Director of Health & Wellness on a timely basis.

19. Organizes and completes assignments on time.

20. Promotes and protects resident’s rights, assists Residents to make informed decisions, treats Residents with dignity and respect and protects Residents’ personal belongings.

21. Recognizes significant changes in the conditions of residents and takes necessary action.

22. Responsible for the detection of and ensuring correction of situations that have a high probability of causing accidents or injuries to residents and staff.

23. Evaluates emergencies and determines emergency measures to be used which are acceptable under assisted living standards and fall within the Good Samaritan guidelines and Nurse Practice Act.

24. Possess leadership and supervisory ability and the willingness to work harmoniously with professional and non-professional personnel.

25. Must possess the ability to plan, organize, develop, implement, and interpret the program, goals, objectives, policies and procedures, etc., that are necessary for providing quality care.

26. Use proper body mechanics when bending or lifting.

27. Maintain current knowledge of the community’s fire safety procedures, including the correct use of the fire alarm system.

28. Able to follow the designated plan of action in the event of a fire or other emergency.

29. Maintain current knowledge and follow the community’s infection prevention policies and procedures.

30. Must be familiar with drug actions and interactions.

31. Attend and participate in in-service educational classes and on-the-job training programs, etc., as scheduled or directed.

32. Perform all other duties as assigned.
