Traveling Behavioral Health Agency looking for a licensed LCSW or LMHC to provide mental health and/or substance abuse counseling at private homes and assisted living facilities in the Jacksonville area. Work schedule: 2-3 days per week; caseload of approximately 40 clients.
Duties include: Providing individual or family counseling, coordinating client care with other team members, including physicians/APRNs, completing chart reviews, evaluating client histories to arrange for appropriate treatment plans, linking clients with community resources as needed, and entering client notes in a SOAP format into computer system.
Maintain confidentiality in accordance with HIPAA regulations.
Minimum of two years’ experience required, ongoing education will be considered. Must be willing to travel and adjust work schedule to meet the needs of the facilities. Bilingual a plus.
Job Type: Part-time
Pay: $25.00 - $40.00 per hour
