We have an opening for Clinical Manager of a busy Medicare certified home health agency located in Broward county. Ideal candidate must have experience in supervising field clinicians, creating and editing plans of care and modification orders. Knowledge of Medicare guidelines for Homecare. Able to manage complex cases involving multi disciplines. Understands the OASIS comprehensive assessments and how to incorporate them into creation of an episode's plan of care. Working knowledge of ICD 10 coding would be a plus. Software used: Kinnser/ WellSky.
The Clinical Manager is responsible for:
- Ensuring that patient care is coordinated and managed appropriately.
- Ensuring that care and services are delivered appropriately.
- Supervision of clinical personnel.
DUTIES & RESPONSIBILITIES
- Receives case referrals. Reviews available patient information related to case, including home visits, to determine home care needs. Assigns appropriate personnel to case as needed. Conferences with Director of Nursing regarding any questions about an individual’s eligibility for services.
- Reviews and evaluates each case through a variety of means such as clinician report, conferences, record review and the services provided by clinicians; discusses and verifies impressions, instructs and guides clinicians to promote more effective performance and delivery of quality home care services; and is available at all times during operating hours to assist clinicians as appropriate.
- Reviews patient’s medical diagnosis, prognosis, medications, procedures and clinical course.
- Assists clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing plan of care.
- Attends case conference meetings with personnel to facilitate coordination of care and discussion of interdisciplinary group involvement.
- Conducts quarterly record reviews and communicates findings and recommendations to Director of Nursing and personnel..
- Complies with accepted professional standards and principles.
- Performs other duties and activities as delegated by the Director of Nursing.

JOB REQUIREMENTS (Education, Experience, Knowledge, Skills & Abilities)
- Graduate of a accredited school of nursing with a BSN degree is preferred.
- Previous experience in home care setting is a plus.
- Registered nurse with current licensure to practice professional nursing in the state.
- Has excellent observation, good nursing judgment and communication skills. .
- Understands principles of pain/symptom management.
- Complies and maintains current CPR certification.
- Demonstrates commitment to patient care and improving health outcomes, able to coordinate with ordering physicians, facilities, and family as needed.

Pay: $72,000.00 - $80,000.00 per year
