About the Job: Alzheimer’s Community Care has a unique fulltime position, Family Nurse Consultant, where you are supporting the caregiver of Alzheimer's patients and providing a safety net for patients & families in the community. This position is similar to a case manager/care manager where you identify services & resources for the caregiver to successfully keep families together and an Alzheimer’s patient living in their home during the entire course of the disease. This is a full-time position in an administrative office, Monday through Friday 8:00am to 4:30pm. This position also involves initial and annual home visits to complete a psycho-social evaluation
You will be successful in this role if you possess these Key Qualifications:
- Unrestricted Florida RN license or LPN license
- Minimum 3 years' home health or related clinical experience as a nurse;
- Ability to build and sustain successful, professional relationships with patients, caregivers, coworkers and community partners;
- Top notch organizational & time management skills, enhanced by strong attention to detail and high-quality work;
- Ability to work as part of a team or independently as needed in addition to accepting and applying critiques and/or feedback; and
- Strong written and oral communication skills combined with a positive attitude to lead a support group and public speaking.
