Job Summary :

Axiom Rehab is seeking a PRN PT. We treat patients on a one-on-one basis for both home health and outpatient home care services. Axiom is one of the largest providers of home lymphedema therapy in Florida.

Job Responsibilities:

• Perform clinical assessments and identify patient needs and appropriateness of care.
• Document, prepare and complete a plan of care, progress notes, phone orders, and other clinical record documentation on an ongoing basis.
• Revise the plan of care based on evaluation and ongoing assessment data.
• Perform and oversee other team members, when applicable, in the performance of patient care that is consistent with patients’ needs and desires.
• Accepts clinical assignments that are consistent with education and competence to care for patients.
• Plan and provide appropriate treatment for cardiopulmonary, neuromuscular, neurological and orthopedic disorders based on medical referral and therapy assessment.
• Apply concepts of infection control and standard precautions in coordinating/performing patient care activities.
• Meet mandatory continuing education requirements of the agency/licensing board.
• Use effective interpersonal relations and communication skills.
• Promote agency philosophy and administrative policies.
• Demonstrate commitment, professional growth, and competency.

Educational Requirements:

• Bachelor of Science or above in Physical Therapy from an APTA accredited program.
• Current Florida State License as a Physical Therapist.
• Current Florida Driver’s License and auto insurance.
• Current CPR.
• Preferred experience as a Physical Therapist in home health or long-term care setting.
• Good organizational and communication skills.
• Bilingual is a plus!

Pay: $85.00 - $100.00 per hour
