As a National Care Technician, you will be at the forefront of our innovative Hospital at Home care model that allows patients who are clinically eligible to receive acute, in patient-level care in the comfort of their own homes. As a National Care Technician, you will be fully integrated into the clinical teams at all our existing and future health system partners, working directly with patients as well as the clinical teams in the hospital and remotely from an office setting.
This role offers the unique opportunity to work directly with patients and their providers to coordinate recovery from acute illnesses, and develop deep relationships to successfully engage them with the resources they will need to improve their health and quality of life.
As a National Care Technician, you will leverage our technology suite as well as our interdisciplinary network of doctors, nurses, pharmacists, and ancillary service providers to improve the health and well-being of enrolled patients and ease the burden on caregivers.
At Shields at Home, you will:
- Manage patient requests, telephonically or through our technology platform, to coordinate the appropriate care and bring it to the attention of the providers as needed.
- Work with partner physicians to educate patients and their caregivers on the Hospital at Home care model; provide education regarding use of the telehealth system and processes.
- Manage Hospital at Home tools and technology, ensuring systems are properly set up prior to a patient's admission. Also responsible for tracking and ordering inventory as needed.
- Work with partner physicians to assist and identify patient eligibility based on a variety of clinical and operational inclusion / exclusion criteria.
- Build and maintain collaborative professional working relationships with physicians, Medical Directors, clinicians, and the community at large to develop and implement a successful cross-continuum care management process.
- Maintain all required documentation in all interactions with our health systems' patients and the care team.
- Observe trends and any program challenges, and work with team to develop operational improvements to increase quality and satisfaction for patients, caregivers, and physicians.
- Communicate discharge information to other clinical departments or members of the Care Team.
- Assist as needed in recruiting, patient and community outreach and critical incident/complaint management.
- Complete other assigned duties as needed.
Required experience / education:
- High school diploma or G.E.D and Florida CNA certification
- Work requires the ability to exchange information on factual matters. This type of interaction requires courtesy and tact when dealing with patients and their families, visitors and/or hospital staff
- Comfort with engaging providers in sensitive work environments such as the ED.
- Comfort with using technology, learning new software applications, and managing multiple documentation and communication streams simultaneously.
- Ability to provide superior customer service; every decision needs to be made with the patient in mind. Patient safety is our number one priority.
- Strong attention to detail and ability to maintain written records and reports. Excellent verbal and written communication that is clear, well-organized, and demonstrates an understanding of audience needs.
- Exhibits exceptional interpersonal skills and proper judgment when dealing with provider partners, staff, and patients.
- Ability to predict needs as best as possible and stay one step ahead of the patient's care.
- Pursues activities with focus and drive, defines work in terms of success, and can be counted on to complete goals. A positive outlook and a can-do attitude are a must.
- Enjoys working within teams, dependable in both productivity and attendance.
- Ability to consistently communicate with teammates; open to opinions and feedback from team members and follows through on all commitments.
Location and travel expectations:
- The role will be based in Tampa, but will require weekly travel (Monday-Friday) within your region and assigned location. You may also be deployed for long-term coverage situations in existing hospital partnerships as unique business needs arise.
