We are looking for a Home Health Registered Nurse (LPN or RN) to join our fast-growing team. Become part of a notable company and help us achieve our goals!

POSITION: Registered Nurse or Licensed Practical Nurse (Home Health)

TYPE: Part-Time nights Friday, Saturday and Sunday

PAY: $25-$45 per hour

LOCATION: Lutz FL

POSITION SUMMARY:

Our RNs and LPNs provide direct patient care to our patients under the supervision of a physician. Responsibilities include following start of care plans, providing treatment, discharges, wound care, medication administration, IV, Tube feeding and working collaboratively with a clinical team to achieve positive patient care outcomes. You will be responsible for educating patients and their families on prevention and healthy habits. Ideally, as a well-trained professional, provide care with little supervision, be compassionate, and follow health and safety guidelines. ESSESNTIAL FUNCTIONS:

• Consult & develop treatment plans based on physician's orders and initial patient assessment.
• Submit regular patient care progress reports to attending physician and clinical staff.
• Provide nursing care and written care plans.
• Participate in and monitor patient and family care compliance activities.
• Professionally manage and coordinate the plan of care
• Complete clinical and medical documentation on all patients.
• Monitor patient’s condition and assess their needs, to provide the best possible care and advice
• Observe and interpret the patient’s symptoms and communicate them to the physicians
• Should have experience with the following: IV experience, pediatrics/young adults, Trachs, G-Tubes, wound care, vent experience preferable, strong clinical experience,
• Must have critical thinking skills, strong clinical background
• Provide immediate medical care in emergencies
• Inspect the facilities and act to maintain excellent hygiene and safety (decontaminating equipment, sanitizing surfaces, preparing beds, etc.)
• Foster, a supportive and compassionate environment to care for patients and their families
• Must be able to work two locations, Lutz and Tampa (15 mins apart)

REQUIRED EXPERIENCE:

• Minimum of Associate degree (or higher) in Nursing.
• Current and unrestricted Florida RN/LPN license.
• Current CPR/BLS certification.
• Minimum of 1 year RN experience in an institutional setting. Acute Care/ Rehab / Hospital /Home Health /Hospice.
• Excellent working knowledge of nursing care methods and procedures
• Team player with excellent communication and interpersonal skills
• Be dependable, reliable, responsible, patient, and compassionate
• Outstanding organizational and multi-tasking skills
• Excellent problem-solving skills

Credentialing Documents to be presented at time of application: Resume, Professional License, Driver’s License, Social Security Card, CPR Card, Physical (within the last 6 months), TB Test (within the last year), COVID Vaccination or Exemption Letter, All Applicable CEU’s
