Our Home Health agency is currently looking for experienced, talented and dynamic Intake Coordinator.
- Experience and knowledge of Home Health intake coordinator required.

About this Position:
- Evaluates patients referred, identifies insurance coverage, and coordinates referrals for services.
- Works with physicians, nurses, social workers and discharge planners to establish a coordinated plan of care.
- Obtains and documents prior authorization for services from insurance providers.
Required Skills:
- Experience and knowledge of Home Health intake coordinator required.
- Minimum of one year of experience in intake coordinating.
- Current AHA CPR certification.
- Self-starter who enjoys working in a fast-paced environment with the ability to multi-task.
- Excellent problem-solving, observation, written and verbal communication skills.
