Home Health Hospice RN Pay Range: Competitive and negotiable market pay, commensurate with experience. Please note* - pay is dependent on location and will vary.

Home Health Hospice RN Schedule: Full-time (40 hours), Mon-Fri schedule

Home Health Hospice RN Responsibilities:

• Member of the interdisciplinary team who work closely to meet the physical, psychosocial and emotional needs of hospice/homecare patients and families

• Provide coordination of care and ensure continuous assessment and implement the Plan of Care

• Appropriately identify referrals such as PT, OT, Home Health, Dietary, Pharmacy, Supplies, etcetera

• Provide education and training to patients, families and caregivers on recommended care and pain management

• Actively participates in patient care meetings and bereavement support

• Serve as the patient and family advocate

Home Health Hospice RN Qualifications:

• Two years of nursing experience as an RN required, any hospice or home health experience strongly preferred

• Active RN license in the state of practice

• Must have a reliable means of transportation

• Ability to lift up to 20 pounds, stand for extended periods, crouch, and bend repetitively

• A strong, dedicated work ethic

• Strong written and verbal communication skills
